Below follows the preliminary program of the event. The event will be in Italian language.
The Marche Region, through the Regional Health Agency, is participating in the European Joint Action JADECARE, aimed at addressing the challenges of healthcare transformation in the European Union. The 3-year project involved 45 organizations from 16 European countries and is going to finish in September 2023. In this context, Marche Region implemented a good practice developed in Spain (Catalonia), focusing on chronicity management through digitalized systems. An innovative algorithm using administrative health flows to assess the health status of the Marche population and the distribution of chronic diseases has been successfully tested. The results of the algorithm will be made available through a dashboard that can explain and show the citizens’ health demand and support the planning of social and health care services. The management of chronic diseases will be promoted through integrated, personcentered care, allowing and efficient use of the available resources, providing the quality of care, and an improved quality of life for citizens.
